Required Documents

Any event that has food provided or sold, food trucks, carnival games, entertainment, inflatables, pony rides, petting zoos, professional groups must submit the required documents no later than 2 weeks prior to the event. They will need to submit any fees associated upon their application request.

All Documents Required Unless Noted

  1. Certificate of Liability Insurance on the ACORD Universal Form naming the Ridgewood Board of Education, 49 Cottage Place Ridgewood NJ as additional insured. Limit of Liability must be at least a combined single limit of $1,000,000 per occurrence/ $2,000,000 aggregate ensuring licensee against any liability for bodily injury and property damage, as well as automobile liability and workers comp.


    Any Vendor that does not have Automobile Liability or Workers Compensation on the COI must sign a Hold Harmless Agreement.

  2. Ridgewood-ODSE Guideline (submitted to ML Coordinator two weeks prior to event; copy to Village Health Department)
    All Group Managers must complete a Ridgewood-ODSE Guideline if there will be food provided, sold or given away. The form does not need to be completed if serving take-out pizza, individually wrapped pre-packed foods (chips, candy, etc.) popping fresh popcorn onsite. Gloves must be worn and changed when handling ready-to-eat foods to avoid bare hand

    contact.
    Copies of the Request for Fire Permit and CATERER-Health Dept ODSE application and Ridgewood ODSE guideline.
    If any event has parent volunteers making or purchasing food contact the Central Office 201-670-2700. You will then be sent a google form that will need to be completed by the parent volunteer.

  3. Application for Fire Prevention Permit - Permit (submitted to Fire Prevention Bureau two weeks prior to even with required fee two weeks prior to event) $54.00 FEE For a truck like Kona Ice Cream & Sweet Escapes, or any food trucks that do not use open flame or grills to cook food do not require a fire permit. The other food trucks – any food truck that cooks in the truck or uses propane in any capacity – needs to fill out a permit application any time they participate in any event. Any vendor using a generator – needs to fill out a permit application anytime they participate in anything. (see downloadable package for form)

  4. CATERER- Health Department ODSE Application - (submitted to Village with required fee two weeks prior to event; copy to ML Coordinator) $50 FEE - If the organization is a nonprofit, they must send an email to the Village Manager requesting a waiver of the fee All vendors will need to notify Health and submit the ODSE application. Vendors can submit their paperwork directly to the Village Health Department. (see downloadable package for form)
